Use this form to request a review of property information, building data, sketches, value, damage, or other property-related records. You may select one or more review options to help staff evaluate your request.

If you select Report Damage, it must be selected by itself. If property damage is the only reason for your request, you may go directly to the Property Damage Survey to complete your request there.

Please be aware that under Florida law, your contact information such as e-mail addresses and phone numbers are subject to public records request (Ref. Florida Statutes 668.6076, F.S.)

Whoever knowingly makes a false statement in writing with the intent to mislead a public servant in the performance of his or her official duty shall be guilty of a misdemeanor of the second degree, punishable as provided in s. 775.082 or s. 775.083.
